Includes only evaluation, completion of physical form, and printing of the vaccination record if needed. Additional services including immunizations will be additional.

• In-office test: COVID/Flu test, RSV test, Strep test, urine pregnancy test, urine dipstick test, fingerstick glucose test
• Medications: Ondansetron (zofran), prednisone. (Ibuprofen / acetaminophen / other OTC drugs do not count)
• Micellaneous: Ear wick placement, hot/cold pack, basin, visual acuity testing, silver nitrate stick, steri-strips

Includes standard services and up to two from any category below:

• In-office test: electrocardiogram
• Injectable Medications: Ondansetron (zofran), diphenhydramine (benedryl), epinephrine, dexamethasone (decadron), methylprednisolone (solumedrol),  ceftriaxone (rocephin),  ketorolac (toradol)
• Procedures: Nebulizer treatment, Cerumen (ear-wax) removal, foreign body removal from ear or nose

Includes advanced services plus:

• Procedures: Abscess incision and drainage, skin biopsy, embedded foreign body removal, IV therapy, nail repair or removal, splinting (X-ray not available), laceration repair
• Depending on the extent of complex services required, additional fees may apply.

These costs are determined by the lab that will be obtaining/performing the lab test.

Tetanus (Tdap), influenza, and others.

Nurse visit for placement of PPD. Return for reading of the test is included. Additional testing/evaluation may be needed depending on your situation.
