Includes evaluation and management by video meeting and/or phone. Duration and complexity of the visit determine cost.

Includes only evaluation, completion of physical form, and printing of the vaccination record if needed. Additional services including immunizations will be charged separately.

Straightforward visit for basic evaluation. (Not common, most visits will not qualify for this visit type)

Evaluation and management for one acute illness/injury or one stable chronic illness.

Evaluation and management typically for one unstable acute or chronic illness/injury or two stable chronic illnesses.

Evaluation and management involving a severely unstable chronic condition or one acute illness/injury that may be life-threatening if not managed.

Includes IV access procedure and initial IV fluids administered. Medications additional.

Add-on additional infusions. Medications additional.

Administration of intramuscular or subcutaneous injection of medication or vaccine OR intravenous push of medication. Medication and vaccines additional.

Includes procedure for repairing a laceration. Complexity and size determine cost. Medications not included.

Includes removal of sutures or staples if not placed at this office. (Removal is included in the initial procedure if performed by us.)

Includes procedure for drainage of abscess. Medications not included.

Venipuncture for blood draw. (IVs listed separately)

Includes dipstick urinalysis for UTI and other purposes. (Microscopic examination and culture can be sent out and billed by Labcorp/Quest)

Throat swab test for Group A Strep

One nasopharyngeal swab that tests for Influenza A/B and COVID-19.

Obtaining and interpreting electrocardiogram. Discounted when combined with school / sport physical.
